VCAR Supplemental Form - To be used for unincorporated protected cells, separate accounts, and filings for insurance subsidiaries, unless required to file on NAIC forms.
[Note: Version 2023 VCAR templates should be used for the 2024 year-end and 2025 fiscal year-end filings, and until superseded.]

IMPORTANT NOTE FOR FILERS:  In order to successfully submit Excel files to DFR:  1) make sure macros are DISABLED on any type of Excel file and 2) do NOT try to submit Excel files with the .xlsm extension.
